The club, who have been relegated to the Championship, tweeted the news on Wednesday.
The tweet said: “Saturday’s mascot is the cover star of the match programme for final home game of the season… what do you think Bradley?”
Bradley is expected to walk out alongside his best friend and Sunderland striker Jermain Defoe at the Stadium of Light.
The pair have struck up a bond with the England forward visiting Bradley regularly as he battles his disease.
